When you use the appointment request form, we collect your name, phone number, email address, the reason for your visit, and anything you choose to add in the notes field. We ask you to keep detailed medical information off that form; the front desk takes the rest of your history by phone.

The front desk uses your information to call you back and schedule an appointment. We do not sell or rent your information to third parties, and we do not use it for advertising.

This website does not use tracking cookies or analytics scripts.

This site links to our Google Maps listing and Facebook page. Those services have their own privacy policies, which we do not control.
