Before you come in
You will complete paperwork, your surgeon will examine you and take films if they are needed, and you will leave with a plan. Sometimes we operate the same day.
Bring with you
- Your referral slip and any films/x-rays your dentist took
- A list of your medications, with your physician’s name and number
- Your completed insurance forms, so the claim is not delayed
- A parent or guardian, if the patient is under 18
Tell us about
- Pregnancy, and any chronic or recent illness
- Heart valves, joint replacements or other implanted devices
- Any difficulty with anesthesia in the past
Fees and insurance
- Credit cards, CareCredit, cash and personal checks
- The front desk handles insurance and billing, and bilingual staff are available
- Ground floor, parking outside the door, and reachable by public transit
